According to the report, the zoo keeper was performing an afternoon feeding when the bear escaped it’s primary enclose. This happened as a result of a door that was left opened.

Action News Jax first told you last month about the attack. That keeper was hurt but did survive.

The bear was killed by the zoo’s response team.

Last week, the zoo announced it is planning security and safety changes following the attack.

The zoo said it plans to install two-locks, two-key systems on the bear exhibit.

They already use these kinds of locking systems on the tiger and lion enclosures. The two-key system requires two people to open, so there’s a built-in, double-check.
